Summary

The Primary Care Provider (Nurse Practitioner/NP) provides comprehensive clinical care - consultation - and patient management in support of the VA mission. They practice to the full extent of their training and certification - follow defined clinical privileges - and hold prescriptive authority for their assigned patient panel or program.

• The Primary Care Provider (NP) is a licensed independent practitioner with advanced education and national certification who provides evidence-based care for a population of patients
• Duties of the position include are but not limited to: Manage a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice
• Develop and implement individualized plans to achieve optimal population health and disease management
• Demonstrate leadership and work collaboratively with Primary Care Team and other interdisciplinary team members
• Order and interpret labs - tests - and diagnostic studies
• Prescribes pharmacologic and non-pharmacologic interventions
• Initiate referrals and consultations to specialty care services
• Provide education to patients and their families
• Assess and adjust the plan of care as needed to achieve optimal patient outcomes

• Influence and model the delivery of evidence-based health care - demonstrate expertise in the ability to manage a patient panel - population - or program

• Acquire knowledge and skills - then disseminate acquired information within and/or outside the medical facility
• Initiate and lead interdisciplinary teams developing systems that positively impact outcomes at the panel - population - or program

• Meet the needs of customers while supporting the medical center and service missions
• Communicate and treat customers (patients - visitors - volunteers - and all medical center staff) in a courteous - tactful - and respectful manner
• Provide the customer with consistent information according to established policies and procedures
• Handle conflict and problems in dealing with the customer constructively and appropriately
